how does medication affect weight loss/gain?

This might be a stupid question, but I’m curious. I’ve heard most people here insist that CICO is the primary / only real factor when it comes to weight gain and loss, but I also have heard so many people talking about medications “making” them gain or lose, and I’m curious how, exactly, that works. Is it that it changes your appetite, causing you to eat more or less? Or is it some sort of complex scientific thing that overpowers CICO? 🤔
